* lisp/hi-lock.el (hi-lock-file-patterns-policy): Add value 'always'.

(hi-lock-mode): Update docstring.
(hi-lock-find-patterns): Return t for value 'always'.
(hi-lock-revert-buffer-rehighlight): Rewrite to correctly handle
all possible cases (bug#57534).
